By visiting the clinic in person
The patient or designated third party by a patient should visit the clinic in person when collecting documents like medical records, medical receipts, prescriptions or doctor's letters. If the patient is requesting release or transfer of protected health information to the third party, the patient needs to follow the procedures outlined below, and the third party should prepare necessary documents.
According to the guideline, we do not send records through fax, email, or mail. When a patient is unable to come in person, a patient can designate the third party to collect their health information on behalf.
